Stimulants — first-line treatment
Stimulants are the first-choice medications for ADHD, effective in 70-80% of patients.
Methylphenidate (Ritalin, Concerta, Ritalin LA)
Blocks the reuptake of dopamine and norepinephrine at the synapse, increasing their availability in the prefrontal cortex — the region central to attention, impulse control and planning.
The extended-release forms (Concerta, Ritalin LA) are preferred to avoid the "rebound effect" late in the day.
Lisdexamfetamine (Vyvanse)
A prodrug converted into dextroamphetamine in the body. It increases the release of dopamine and norepinephrine. The gradual conversion mechanism produces a smoother onset and lower abuse potential than immediate-release amphetamines.
Tends to have a more even effect across the day with less of a late-afternoon drop-off.
Non-stimulants — alternatives
Used when stimulants are contraindicated or insufficient.
Atomoxetine (Strattera)
A selective norepinephrine reuptake inhibitor. Not a stimulant — no abuse potential. Full effect takes 4-8 weeks. An option when stimulants are contraindicated (severe anxiety, history of substance abuse, certain heart conditions).
Guanfacine (Intuniv)
An alpha-2 adrenergic agonist. Improves attention and impulse control through a different mechanism than stimulants. Often used in combination or as an alternative. Its sedative side effect can be a benefit in children who also struggle with sleep.
Myths about ADHD medication
"Ritalin is addictive"
When taken as prescribed by someone with ADHD, methylphenidate does not create dependence. Untreated ADHD, by contrast, raises the risk of substance dependence — medication actually lowers that risk.
"Medication changes a child's personality"
Medication reduces ADHD symptoms, not personality. Parents often describe the child as having "become themselves" — less frustrated, more able to show who they really are without the obstacles ADHD creates.
"Only children take ADHD medication"
Adults with ADHD benefit just as much as children do. Late diagnosis is common — many adults are only diagnosed after 30 or 40 and report that medication changed their life.
"Medication replaces therapy"
Medication and psychotherapy (especially CBT) work on different fronts and complement each other. Medication improves the capacity to sustain attention; CBT teaches organizational skills, emotional regulation and changes to behavior patterns.

Who can prescribe ADHD medication?
Prescribers vary by country — typically psychiatrists, developmental pediatricians (for children), neurologists, or in some places a primary care physician experienced in ADHD. Stimulant medications are controlled substances in most places, which means prescriptions cannot simply be renewed indefinitely without follow-up. Ongoing specialist involvement is generally recommended for dose adjustments.
Does ADHD medication need to be taken forever?
It depends. Some adults with ADHD benefit from lifelong continuous use. Others go through periods of use and pauses. "Medication holidays" on weekends or school breaks are common in children when the main benefit is academic. The decision is always individual and made with a doctor — it should not be stopped abruptly.
What are the most common side effects?
Stimulants: reduced appetite (especially at lunch), difficulty sleeping if taken late in the day, a mild rise in blood pressure and heart rate, initial headaches, irritability during the "rebound". Most effects are dose-dependent and can be managed by adjusting the dose or timing.
Does ADHD medication work in adults?
